Loan Repayment Programs

After graduation, several programs can help manage student debt:

Public Service Loan Forgiveness (PSLF)

Full forgiveness of remaining federal Direct Loan balance after 120 qualifying monthly payments. PTs working full-time for government agencies, VA hospitals, nonprofit healthcare facilities, and public schools typically qualify. Forgiveness is tax-free, and payments do not need to be consecutive. Note that new rules taking effect July 1, 2026 may narrow which employers qualify.

VA Education Debt Reduction Program (EDRP)

Up to $200,000 over 5 years (up to $40,000 per year, tax-free) for PTs hired into VA positions designated as difficult to recruit. Can be combined with PSLF. Apply within 6 months of appointment.

Indian Health Service Loan Repayment

Up to $50,000 for an initial 2-year commitment ($25,000 per year), with annual extensions available until debt is fully repaid. Physical therapists are explicitly listed as an eligible health profession. Priority given to American Indian/Alaska Native applicants.

IHS Scholarship Program

Up to 4 years of full funding (tuition, fees, living stipend) for American Indian and Alaska Native students who are members of federally recognized Tribes pursuing health professions degrees. Requires a minimum 2-year service commitment at an Indian health facility.

HRSA Faculty Loan Repayment Program

Up to $40,000 over 2 years for health professions faculty from disadvantaged backgrounds with a 2+ year teaching contract. Your institution provides matching funds.

State Loan Repayment Programs

Many states offer their own loan repayment for PTs serving in underserved areas. Examples include Missouri HPLRP (2-year service commitment), Montana SLRP (up to $50,000), and California AHSP (up to $8,000 with 12-month service obligation). Check your state health department's website for local programs.

A Note on NHSC

Physical therapists are not currently eligible for the National Health Service Corps Loan Repayment Program. Legislation to add PT eligibility (H.R. 5621, the Physical Therapist Workforce and Patient Access Act) is pending in Congress. Monitor APTA advocacy updates for progress.
